Remember how I was ranting about the use of "and/or" in the pseudolegaleze bullshit I was transcribing?
That was NOTHING.
This, I promise you, is VERBATIM and UN-EDITED, except the company name was removed. The document is titled "Accident Investigation".
Do they possibly mean "investigated" instead of "conducted"? The world may never know, because whoever wrote and PUBLISHED this document subsequently had their license to use the English language revoked. (It still wouldn't really make sense if the right verb were in it, but at least it wouldn't directly contradict the conventions of reality.)
