Here's a hilarious one:
I didn't get the bill from Univera to pay our health insurance when we enrolled until after it was already overdue.
So they cancelled our health insurance. Made us pay three months to sign back up.
We had to pay for August-- of course-- and October-- of course.
We also had to pay for September.

During which month we expressly had no coverage.

Yes. They have forced us to pay them for a month's worth of health insurance, for a month in which we were denied coverage.

I LOVE THIS. How can I found my own insurance company? I urgently need to go into this business. I need to do this. You can charge people full price for a whole month in which they are expressly forbidden from actually using any of the shit they are paying for!!

Insurance companies are evil. I just wrote mine a nasty letter for rejecting some of my claims over the past year; they took so long to cash my post-dated cheques that they ended up cancelling my coverage every month.
